Sunday night's mistake at the Oscars will likely be remembered as the biggest flub in the event's 89-year-history, but it's not the first time it's happened. At the 36th Academy Awards in 1964, Sammy Davis Jr. was handed the wrong envelope in presenting the award for Best Music Score. He announced the winner as John Addison for Tom Jones, but he wasn't even nominated in that category.
